Have you been diagnosed with Rheumatoid Arthritis (RA) by a Rheumatologist? Yes No
Are you currently on Methotrexate for treatment of your Rheumatoid Arthritis? Yes No
Are you currently taking a biologic medication for your Rheumatoid Arthritis (i.e. Humira, Enbrel, Rituxamab, etc) Yes No
Have you had any prior physician diagnosed cardiovascular event (i.e. MI, angina, stroke, TIA)? Yes No
Have you had any prior cardiovascular procedure (i.e. CABG, angioplasty, valve replacement, pacemaker) Yes No
