Additional Study Information: This study is to collect information about the symptoms and treatments of patient who have severely clogged arteries and to learn about their outcomes at one year. If you choose to be part of this research, we will ask you to be involved in five interviews with study staff. Each interview will take about 30 minutes of your time and we will contact you annually for 5 years. Your involvement in the research will last for 5 years. There are no or minimal risks from participating in this study. There are no physical risks associated with this study. We will take measures to maintain the confidentiality of all study participants, however, accidental disclosure of your medical information is a potential risk. The study may have no benefits to you directly. You have the alternative of not taking part in this study.
