Additional Study Information: This patient case registry for Lyme and other Tick-borne Diseases is a component of the Clinical Trials Network that was launched in 2021 by a grant from the Steven and Alexandra Cohen Foundation. The registry will gather general information such as demographics, medical history, and current functional status. This information will help researchers to identify individuals who may meet criteria for current or future research studies. It will also help us to share news and updates about ongoing clinical research. Individuals in the registry may receive notifications from the Columbia research staff about studies that may be of interest to them. In addition, participants will be asked to provide an annual update on their clinical status to allow the Columbia research team to assess clinical change over time among registry participants.
